Request for Insertion Forms

Non-federal government clients

As a non-federal government client, i.e. provincial, territorial and municipal governments, law firms, companies, non-profit organizations and individuals from the private sector, you are asked to submit a request for insertion form each time you publish a notice in the Canada Gazette. However, if you submit several notices to be published on the same date, only one form is required. Please ensure that each notice is correctly identified on the form. For more information, please refer to the steps for Non-federal Government Clients.

Note that the HTML form has been deleted and that the PDF version is now only available and is consistent with accessibility requirements. You must complete the PDF form, print it and send it to us along with your notice.

Should you require assistance completing the form, please consult the Guidelines for completing the request for insertion form for non-federal government clients.

To read the PDF version, you must first install PDF reader software. A list of free downloadable software is available on the Public Services and Procurement Canada website.
